Ovarian cysts and Laparoscopic Treatment

Ovarian cysts are becoming more common, particularly among women of reproductive age.


Types of Ovarian cyst


Functional cysts; These cysts are considered normal and do not need treatment.

Endometriotic cysts are blood-filled cysts that may cause severe discomfort during periods, persistent pelvic pain, and infertility.


Traditionally, these cysts are treated with open surgery/laparoscopy and removal of the cyst wall, but many times the surgeon accidentally removes a large amount of normal ovarian tissue while peeling out the cyst wall, resulting in a loss of ovarian reserve and the ability to have children before her time.

To prevent this doctors now have Carbon dioxide Lasers that drain the blood from the cyst and treat the cyst wall with the laser rather than taking it out.


This procedure has significant benefits in that it eliminates needless ovarian loss and, since it is performed laparoscopically, it may be completed in one day with minimum blood loss and no or minor discomfort.


Polycystic Ovarian Condition


Increasingly prevalent among young women, this disease causes infertility, hirsutism (male-like hair on the face, breast, back, and abdomen), and monthly abnormalities.


Usually, this is treated with diet, exercise, and oral medications, but in some refractory cases, it is treated with laparoscopy, where these cysts are broken again. However, over-drilling of these cysts by the surgeon can lead to loss of ovarian reserve, so gentle use of Co2 Laser is found beneficial in these cases.


Dermoid cysts


These cysts, which include fat, hair, and occasionally even teeth, are highly prevalent in women of reproductive age. They may also undergo a malignant transformation in 2% of cases.


Also, these cysts commonly twist on themselves, resulting in full ovary loss (Torsion). These are treated by complete laparoscopic excision in a bag.


Germ cell tumors and other cancers


When germ cell tumors, such as dysgerminoma, occur in young women who desire to have children in the future, the tumors can be removed laparoscopically  (to prevent tumor cells from spilling) while preserving her uterus and another ovary for future pregnancy. The woman is then given radio/chemotherapy.


Because there are various reasons for a cyst and inappropriate treatment might result in ovarian reserve loss or partial cancer removal, doctors urge that ovarian cysts be treated by professionals in order to avoid these complications.
